Aphasia Intervention Strategy: ( ORLA ) A Practical Approach for Improving Communication skills.


 

Introduction.

ORLA or Oral Reading for  Language in Aphasia is a reading treatment for people with aphasia. It is focus on reading full sentences rather than single words. It is an evidence – based practice. ORLA is a structured therapy technique  to provide increasing levels of difficulty and allowing patients to gradually develop confidence and skills .ORLA originally develop for target to reading comprehension skills ,but it also beneficial for increasing auditory comprehension ,fluency ,prosody ,memory and other aspect of speech and language.




Participation population.

People (adults) with mild to moderate chronic fluent and non-fluent aphasia such as Broca’s aphasia, who have intact literacy skills to increase receptive language.

ORLA may be too advanced for people who have severe comprehension or reading impairments.


Pre-requisites or contraindications .

Patient should have some ability to match words to spoken words.

Person who has severe cognitive impairments, significant visual deficits , profound auditory  comprehension impairments may limits effectiveness using ORLA.

And also person who had sever apraxia of speech may also be a contraindication.

the techniques can be used with any text. For instance, an SLP can select a news article or any text about a topic of interest


 

Sentence

 

Paragraph.

A brief description of the steps involved in technique .

            HOW TO DO THE  ORLA.

Step 1

Find a stimulus sentence or paragraph .



Step 2

Therapist read aloud to the patient, articulating  carefully.  The therapist’s mouth is visible to patient.

Step 3

The therapist reads aloud to the patient points to each word as it is read.

Step 4

 Therapist read the sentence aloud again .This time ,patient point to each word as it is read and attemps to read each word aloud in unison with the therapist.

Step 5

Doing the step 4 again ,this time the voice of therapist fades out at the end of the sentence. It helps to patient to read the word aloud independently.

Step 6

For each line or sentence , the therapist ask  patient to point to  the word what therapist said .(These words can be noun ,verb or functional words.)

Step 7

For each line or sentence ,the therapist points to a word for the patient to read aloud.

Step 8

The patient reads aloud the whole sentence aloud again in unison with therapist.

   Home-based activity based on the objectives/principles of the ORLA.

  • ·       Select the appropriate material (choose sentence or short paragraph according to his reading level and his or her preference.

  • ·       Do the same steps in ORLA with care giver. Like caregiver read aloud text , read with patient together and point target words .
  • ·       Given time for  patient to reads the text alone ,with assistance as needed.
